2008 Lexus RX400h – ABS Brake and TRAC Lights With C1391
CUSTOMER CONCERN Customer states the ABS, brake and traction-control warning lights are on. DTCs C1241, C1256 and C1391 are stored. CAUSE Accumulator-pressure voltage dropped after the pump shut off, confirming internal leakage in the ABS brake actuator assembly. CORRECTION Remove and replace ABS brake actuator assembly. Perform actuator initialization and brake-system bleed. Clear brake DTCs.
2008 Lexus RX400h – No Start With P0351
CUSTOMER CONCERN Customer states the engine will not start and DTC P0351 returns immediately after clearing. CAUSE Lab-scope testing of the IGT command and IGF feedback circuits confirmed a failed ignition-coil assembly. CORRECTION Remove and replace failed ignition-coil assembly. Clear DTC P0351 and verify normal starting operation.
2008 Lexus RX400h – Engine Temperature Fluctuates With P0117
CUSTOMER CONCERN Customer states the engine-temperature reading fluctuates while driving and DTC P0117 returns after coolant-temperature sensor replacement. CAUSE The 5-volt reference and warmed-up sensor signal tested correctly, confirming the radiator was not maintaining stable coolant temperature. CORRECTION Remove and replace radiator. Refill and bleed engine cooling system, clear DTC P0117 and verify stable coolant temperature.
2008 Lexus RX400h – Overheats and Heater Blows Cold
CUSTOMER CONCERN Customer states the engine overheats while driving and the heater blows cold. Coolant temperature reaches approximately 230°F with the cooling fans operating. CAUSE Cooling-system bleeding and flow checks confirmed combustion leakage from a failed cylinder-head gasket. CORRECTION Remove and replace failed cylinder-head gasket. Refill and bleed engine cooling system and verify normal operating temperature.
2007 Lexus RX400h – Hybrid Warning With P0A93 P0A05 P0A08
CUSTOMER CONCERN Customer states hybrid-system warning lights are on. DTCs P0A93, P0A05 and P0A08 are stored. CAUSE No coolant movement was visible in the inverter reservoir and the inverter coolant pump did not operate, confirming loss of inverter-coolant circulation. CORRECTION Remove and replace inverter water pump. Refill and bleed inverter cooling circuit. Clear hybrid-system DTCs.
2007 Lexus RX400h – ABS Pump Runs With C1391
CUSTOMER CONCERN Customer states the ABS and brake warning lights are on and the ABS pump runs repeatedly. DTC C1391 is stored. CAUSE No external brake-fluid leak was found, and accumulator pressure dropped after pump shutdown, confirming internal leakage in the ABS brake actuator. CORRECTION Remove and replace ABS brake actuator assembly. Perform required actuator initialization and brake-system bleed. Clear DTC C1391.
2007 Lexus RX400h – No Brake Pressure After Caliper Replacement
CUSTOMER CONCERN Customer states brake pressure was lost at the right rear after rear brake and caliper replacement, and conventional bleeding did not restore the pedal. CAUSE Air remained trapped in the electronically controlled brake system, leaving insufficient hydraulic pressure for conventional bleeding. CORRECTION Use a compatible scan tool to operate the brake pump, build hydraulic pressure and perform the complete brake bleeding procedure.
2007 Lexus RX400h – ABS and Traction Lights With C1241 C1249
CUSTOMER CONCERN Customer states the traction-control, ABS and brake warning lights are on. DTCs C1241 and C1249 are stored. CAUSE The stop-lamp input circuit did not supply battery voltage to skid-control ECU connector S39 terminal 14 on the Black wire with the brake pedal applied, confirming a failed brake light switch. CORRECTION Remove and replace brake light switch. Clear brake-system DTCs and verify stop-lamp operation.
2006 Lexus RX400h – Check Engine Light With P2603
CUSTOMER CONCERN The check-engine light was on with DTC P2603 for the transaxle oil-pump control circuit. CAUSE The transaxle oil-pump motor was confirmed faulty. CORRECTION The transaxle oil-pump motor was replaced.
2006 Lexus RX400h – No Start With P0351
CUSTOMER CONCERN The engine would not start and DTC P0351 returned immediately after being cleared. CAUSE The ignition-coil control circuit wiring was faulty. CORRECTION The damaged ignition-coil wiring was repaired.
2006 Lexus RX400h – Starts Then Dies With Timing Codes
CUSTOMER CONCERN The engine became noisy, stalled, and would run only briefly while storing P0016, P0018, P3190, P3191, P3193 and P0A0F. CAUSE The valve timing was incorrect because the timing belt was misaligned. CORRECTION The timing belt was correctly aligned and engine timing was restored.
2006 Lexus RX400h – ABS, Brake and TRAC Lights On
CUSTOMER CONCERN The ABS, BRAKE and TRAC warning lights were on with DTCs C1241, C1256 and C1391. CAUSE Accumulator-pressure data confirmed an internal brake actuator fault. CORRECTION The ABS brake actuator assembly was replaced.
